Considering the timing, I suspect he had taken the mRNA vaccines and developed heart problems because of it.

Respect to ICP for dispensing the proto clown pills but life eventually catches up with you. And if you had to ask him I bet he doesn't regret a thing except that he didn't do more of all of it..
He's also been really fat forever. Depending on what substances he enjoyed (if he did), that could've contributed. Of course, it really could've been the vaccine.
