At one point, the British nuclear arsenal used a nuetron absorbing wire inserted into the core as a safety device. A motor driven spool would pull the wire out just before launch. The combination of underpowered motors and corroded wires fusing with the core meant the entire arsenal was merely dirty bombs. And, the Brits were still generally competent at that time.
