This is why people in the era carried on an empty cylinder. But on a modern reproduction? Yeah sure something could get by QC (see anything made by Keltec) but pretty unlikely unless it was made by Taurus.

Even modern New Frontiers and SAA repros should be carried hammer down on an empty chamber.
Ruger Blackhawks however have a transfer bar safety that obviates this.
Oddly old cap and ball Colts like the 1860 have safety spuds between the nipples allowing for safe carry with all 6 chambers loaded.
