Was THAT the cause? Any fucking prop guy prior to 1999 could have made a afro helmet in a couple of days! The bigger the fro the easier the task. I don't think Reynolds or Dominos' actress aught to be blamed, but they could have called out these bad decisions at some point, even years later. But again, there's a long Hollywood tradition about not giving a shit the working schleps like stuntmen.
Yes, they won't admit it but there is zero other reason why a fucking stunt actor on a motorbike wouldn't be wearing a helmet and at the time there was a lot of side talk around why she wasn't that alluded to the afro being the reason. On top of her inexperience and gross overconfidence.
I blame Domino's actress because she accepted the role knowing she was being used as a diversity hire and political tool, cashed their money happily, and helped directly lead to the death of that woman who was impersonating her. She had more control than most of the factors that lead to what happened by her inaction and willingness to be a blackwashing tool.

She was a motorcycle racer and this was her first job in stunts. So while she was technically experienced, the bike they used for the movie was way stronger than she was used to and they just forgot to set up barriers to keep her from flying off set, which is how she would end up crashing. Because they outright admitted in court to having basically given zero fucks to safety that day.
Which they knew she wouldn't be ready, because they imported a random black racer from Brooklyn all the way to Vancouver "for some reason" (we know why).
