He was clearly just tossing the gun around with little care for handling. Guns are pretty well designed to your finger comfortably going to the trigger when you grip it.

So its almost guaranteed he just wasn't paying enough attention, gripped it harder to reinforce his grip and it was at the exact wrong moment when it was pointed at his head.
