I never understood why a distinction is made between porn stars and prostitutes.

Two differences that come to mind:

Prostitutes have sex with more, poorly vetted people and thus are more likely to spread disease.
Porn stars and their viewers don't cause the same social decay that johns and streetwalkers trawling the streets do.
