Civil wars occur when a powerful faction has its interests threatened, not because the state is acting immorally, which it almost always is. If the state started killing people who count, that might cause a civil war. Fetuses don't count, as they do not have any power.

If there was a state-sanctioned killing of tens of millions of 6 year olds over decades with no sign of stopping, I 100% believe arms would be taken up.

Moral indignation does not exist. Any response would be because people would fear for their own children, or grandchildren, or for the societal consequences of it.
People just don't treat fetuses morally like 6 year olds in practice (which is why it pisses me off that they pretend like they do).
