Miranda literally replaced Mia as his wife for a while, and is shown to be very committed to the role. If I recall right she even mocks him for not noticing because of how well she did. Stands to reason some physical parts happened.

Ethan wasn't the one cucked, it was Mia. And couldn't have happened to a more hateable character after RE7.
