One of the common, and perhaps overused, plot hooks in the 'What If?' stories are "What if X died during this event?", [literally in one case where Professor X died from the Techno-virus], so the opposite where someone lives is often an interesting and yet at times tragic comparison.
The first Civil War event had a version where Cap and Tony don't end up fighting during their initial conversation and many heroes don't die in the resulting conflict. This by itself has severe consequences due to future events but the largely peaceful outcome because of relatively minor changes is soul crushing to Tony because if how things turned out otherwise.
Yea the “what if they didn’t die” story was a common one and things usually ended worse like if Gwen Stacy lived. The one where Spidey marries black cat was interesting. But I’m not surprised they didn’t choose to use the comic books as an inspiration
