Already planning to make my own since the US current form is pretty much a postcard and scribbles. Just in case I need for something. I'm going to wait until later when I would be more realistically eligible to get the vaccine though. Good lies have to be based on the truth.
I'm not even sure I'm against getting the vaccine, but I'm definitely not going to seek it out, particularly with how much of a pain in the ass it is. I already had the virus anyway.
The one I downloaded from here had two sides to it with a form on the back for reminder dates.
I haven't tried to print it yet, but it shouldn't be a big deal. The biggest issue would be either getting the appropriate size card I could print on directly or cutting a full page so it doesn't look like it's cut. Fill it out and then leave it sitting in console of car or something fairly normal to get it "aged" and should be great.

Not something I'm going to try for a couple months, since like I said I want to wait until it would be reasonable that I would have gotten to avoid questions. I'll probably just go for something on a guess with the assumption there will be enough variations and the places I might need it wouldn't be checking in that much detail.

.35mm thick according to a micrometer, which the internet tells me is 110lbs paper.
I forgot how useful micrometers are. The blanks cards I have measure just over .35mm and the internet says that is 110 lbs card stock.
