I hope there is more to this, the way they "caught" him was suspicious to say the least. Still better writing then any modern day show.
Also to note he appears to be from a very wealthy family so the idea that either him or his family lacked medical care just doesn't hold water.
having "wealth" doesn't automatically mean you can spend boatloads of cash, and it wasn't accessing the care that was the problem.
Being rich means he was covered and his immediate family was also covered. If his or someone from his family had problem with their insurance claim then they have lawyers to challenge or even pay up since they can afford it.
There may be some scenario like a friend or girlfriend but without any additional information it is as relevant as aliens. With the information that we have medical insurance should not have been a problem.
I'm still hoping there is something more although it is probably just pure ideologically motivated. You need to understand that this has more suspense and interesting characters that any movie. I'm just waiting for the revelation and understanding what the character arc was for Luigi.
Back surgery. I suspect he was a narcotic addict as is so often the path, and his insurance wouldn’t let him get opiates. Or maybe he blames the healthcare industry for denying opiates to “those who need it,” all of whom became addicts.
