And Russia with Chechnya, Georgia, transnistria, and recently Ukraine. Since almost all of those have now become defacto part of Russia. So far I have yet to see a new star added to the American flag with the Iraq state there...
Because adding a new state comes with responsibilities. Much better to leave them as vassals. That way you also avoid the annoying voices of the veterans from PR that can be abandoned as 'non-americans' when it comes time to pay them benefits.
