If you look at the very early vaccine cards from December there was less consistency in how they were printed. Not everyone was using the "official" CDC ones, some hospitals pre-printed their own so the nurses didn't have to fill everything in by hand, etc...
If a card had a hospital logo on it instead of a CDC logo, it might look unusual but not outside the realm of possibility. "[Shrugs] That's what they gave me"
