Almost never. That's why we still remember the name of so many game studios and specific game creators from the 80s and 90s. Putting together a team is an incredibly time consuming, expensive process. It costs far, far more money to hire a new employee than to retain existing employees.

XCOM 2 is one of my favorite games of all time. I just really hate having a great run destroyed because the enemy pods all sprint directly toward my squad while the game pretends they just coincidentally all happened upon us.
You're very wrong about the accuracy thing, but dead on with the pod mechanic. Inactive pods don't behave in a realistic way, and the turn based system means that being spotted by an alien can mean instant ruin if it happens at the end of your turn. The player shouldn't be punished for taking actions in the wrong order if it's not possible to know what the correct order is.
I can't personally think of a way to remedy it without just removing the "entire team goes at once" mechanic, but it's uniquely frustrating to accidentally reveal the wrong tile and basically give the aliens a free turn. Activating a pod should at least give your entire team free overwatch or something.
TOME is probably the classic example. Insane amount of content, but also well done and not designed to just kill you instantly.
For an older example, you could try ADOM. It's pretty hard, often unfairly so, but it's probably the most fun of the "old as shit" roguelike catalogue.
I've also been looking into one called Quasimorph, but I haven't actually played it yet for lack of time.
Has anyone actually played this? When done well, extremely complex roguelikes offer some of the best, most memorable gameplay there is. But so many of them fall into either the "hard equals good" mindset or they'll just toss the player into the deep end and expect them to learn to swim by spending 50 hours reading the wiki. Neither design philosophy is fun at all to me. I'm really hoping this one finally got it just right.
